Proof of Residency: provide two (2) of the following: Residential Lease, Mortgage Statement, Gas/Electric/Oil Bill, Water Bill, Income tax form, Voter Registration documents, Driver's license, Learner's permit or non-driver identification.
Immunization: (English only, signed by a physician)

Parent Photo Identification: please provide one of the following: Driver's license or Non-Driver's license, Passport, Sheriff's Identification

The Residency Affidavit: (If you live with a LaFayette Resident) needs to be completed and notarized. You can download a copy of the Affidavit from the Registration page on our website. If you require a Notary, please call the Registration Office at 315-677-9728 to schedule an appointment.
